    Please make sure that when you have finished filling out this form you hit the "Submit" button. If you did not fill in one of the 
    required fields the planner will not be submitted and you will be asked to fill in the fields that you have missed.

    Please note that for the reception you will need to supply one 6 or 8 ft. table for your DJ plus one chair. It is always a good idea to have the table covering match the décor in the room. If we are DJing your ceremony we will need one very small table (2’ X 2’ will work fine) at the back of the ceremony area (behind your guests and to one side)

    We would normally DJ the dance until 1 am or earlier. If you want the DJ to play later than 1 am there will be a charge of $100 per hour.

    Locations - This is important as you DJ will need to know where to set up for each event at your wedding.

    Where at the venue or address will your ceremony take place? This is important as we need to know how close the ceremony will be to a power outlet. Please let us know how far an electrical power outlet will be from the ceremony site if further than 50 ft. away.

    Ceremony Information - Only fill in this section if Something Blue is DJing your ceremony

    Besides your music, we will always provide either a handheld wireless mic or a lapel mic for your wedding officiant. If you are having someone do readings at your ceremony we will also provide an extra mic on a stand for them. Please let us know here if you are having someone do a reading at your ceremony by answering yes or no.

    Reception Information:

    Please give us an address that we can find with a GPS if possible. If you need to give us directions please use the box at the bottom of this form
    We need to know this so we know if we need to set up another sound system for the cocktail hour to provide music for your guests. If your DJ is not DJing your cocktails (you may have hired a musician to play during this time) simply write in N/A.
    Simply fill in the genre of music that you would like played during the Cocktail hour. If unsure write "DJs Choice". Keep in mind that this is just background music while your guests are mingling. A long list of song choices is not necessary.
    Who will be introducing the wedding party and the bride and groom into the hall? This is usually done by the MC or the DJ. If you are not doing hall introductions simply fill in N/A here and skip the next few sections
